Download the Program Application form from Institute portal www.icis.co.in Submit the application form along with the program and prospectus fee to the institute office at Noida.

Application Form Requirement:

All applications duly filled in all respects and signed by the candidate must be accompanied by:

• Bank Draft in favor of "ICIS" (Payable at Noida / New Delhi).
• Photocopies of degree / diploma of last qualifying examination with self attested.
• Photocopy of a certificate as to the proof of Date of Birth (preferably the High School or the Higher Secondary Certificate)
• 2 copies of Passport size photographs - 1 pasted & 1 pinned
• Please submit only photocopy of above documents. The completed application form is to be submitted by post / courier at below mention office address to Program Coordinator:-
